Denial of Release of Information/Photography Opt-Out

The district’s policy 515 relating to the protection and privacy of student records is in place to allow minimal information, as defined by the district in accordance with state statute, is available to the public. Should parents/guardians (or the student aged 18 or older) not want the information shared, this form must be completed and submitted to the Office of Student Information or to the office of the school the student attends no later than Oct. 1 each school year.
