State Vaccination Sites

The State of Minnesota has launched the Minnesota COVID-19 Vaccine Connector, which is a tool that helps Minnesotans find out when, where and how to get their COVID-19 vaccine.

School staff who have already completed the Minnesota COVID-19 Vaccine – Education and Child Care survey do not need to complete the Vaccine Connector form or re-submit their information. If you have not already been added to the waitlist, you can sign up for for the Vaccine Connector at mn.gov/vaccineconnector.

County Vaccination Sites

Feb. 24, 2021

Washington County Public Health and Environment has been allocated COVID-19 vaccines for South Washington County Schools (SoWashCo) staff to be administered at Park High School. Park High School will be a county vaccination site for SoWashCo and several other school districts.

Washington County Public Health is using the state’s waitlist to vaccinate residents of Washington County. As a result, SoWashCo staff who live in Washington County will be randomly chosen for this vaccination clinic. Staff who are selected will be contacted directly by Washington County Public Health.

If you are selected:

  • Once you have confirmed your appointment, tell your supervisor so arrangements can be made to cover your job duties for the time needed to get the vaccine. (approximately 25 min. plus drive time)
    • Taking the full or half day off from work is not necessary.
  • If you currently have a vaccine appointment at another site, please continue with your vaccine at that site.

Hy-Vee Pharmacy Vaccination Sites

March 5, 2021

Hy-Vee Pharmacy has partnered with the Centers for Disease Control and Prevention (CDC) to provide the COVID-19 vaccine to Minnesota educators and Childcare workers. Vaccine appointments will be available at our retail pharmacy locations in Minnesota per guidance from the Biden Administration earlier this week.

Please visit Hy-vee's website to fill out the digital vaccine consent form and select a vaccine appointment time and location. Please be patient and do not call your Hy-Vee Pharmacy. They do not have access to schedule appointments that are not available online.

Please bring your school ID/badge, insurance card and a mask to cover your nose and mouth to your appointment.
