Health Insurance

FOR 2019 - THE FOLLOWING CHANGES WILL BE MADE:

Read the information below carefully prior to making your health plan election.

  • New Administrator
    •  Preferred One will be the new plan administrator.
  • Premiums
    •  Click here to access the 2019 Health Insurance premiums.
  • Plans
    • There are no plan design changes, i.e. deductibles, out-of-pocket limits and maximum out of pocket limits.
    • We will continue to have 3 options for employees to select form:
    • Health Insurance with preventative dental benefit only – there will still be a preventative dental benefit under all medical plans with PreferredOne, however most HealthPartners dental clinics are not contracted with PreferredOne.  Thus, please click here to access a list of in-network dental providers within a 50-mile radius of Cottage Grove MN.  PLEASE NOTE: these are preventative dental benefits only; please review our dental insurance section for further dental coverage options.
  • VEBA (for employees enrolled in the Open Access High Deductible plan)
    • Effective January 1, 2019, 121 Benefits will be our new administrator.
    • FSA and VEBA Claims Blackout Period

      If you currently have a VEBA account, please note there will be a blackout period to receive reimbursements, while we transfer the VEBA accounts from PlanSource to 121 Benefits. 

      What does this mean for me?  This means that if you plan on requesting reimbursements from your VEBA prior to the end of this year, you will want to have your reimbursement requests to PlanSource no later than December 14th for the final PlanSource payout on December 21st.  All reimbursement requests after December 14thwill be made to 121 Benefits and will be reimbursed at the end of blackout period in accordance with the 121 Benefits reimbursement schedule.  Your 121 Benefits VEBA welcome packet will include information on their reimbursement process.  We anticipate the blackout to end on or before January 14, 2019. 

      If you currently have a 2018 Health or Daycare Flex Spending Account, please have all of your reimbursement requests submitted to PlanSource on or before December 21st for their final payout on December 28th.    Reimbursement requests after December 21st should be submitted to 121 Benefits and will be reimbursed at the end of blackout period in accordance with the 121 Benefits reimbursement schedule. 

      We do allow a 2.5 month grace period for submitting claims.  Any dollars you have left in your account on December 31, 2018 will be available for reimbursement of claims incurred in 2018 and January 1 through March 15, 2019 from 121 Benefits through March 30, 2019.

Note:  If you do not take action during Open Enrollment, you will remain in the plan you are currently enrolled in.

 


Dental Insurance

FOR 2019 – No Changes to Plans:

Read the information below carefully prior to making your health plan election.

Note:  If you do not take action during OE, you will remain in the plan you are currently enrolled in.

 


Flexible Spending Account (FSA)

FOR 2019 - the following changes will be made:

  • New Administrator
    •  121 Benefits
  • HealthCare FSA
    •  The current Health Care FSA annual maximum is $2,650.
  • Dependent Care
    • The Dependent Day Care FSA annual contribution is $5,000 per family
  • FSA and VEBA Claims Blackout Period

    If you currently have a VEBA account, please note there will be a blackout period to receive reimbursements, while we transfer the VEBA accounts from PlanSource to 121 Benefits.

ACTION REQUIRED:  You must enroll in Health Care FSA and/or Dependent Day Care FSA each year you want to participate.  You enrollment for 2018 will not automatically continue into 2019

Eligible employees will receive an email from 121 benefits with your log in information.

Other Considerations for 2019

There is no open enrollment opportunity for the following plans; you may only enroll through an Evidence of Insurability process, which may require medical underwriting.  If you are interested in completing this process, please contact your benefit specialist for the necessary paperwork.

  • Long Term Disability (LTD), underwritten by The Standard
  • Life Insurance, underwritten by The Standard
  • Supplemental Life Insurance, underwritten by The Standard
