HealthPartners is the current health insurance carrier for South Washington County Schools Eligible employees may choose single or family coverage or elect to waive coverage altogether.  Open enrollment for health insurance occurs every year, so if coverage is initially waived, adding coverage can only be done during open enrollment unless the change coincides with a qualifying family status change or life event. Three insurance plans are offered and summarized below:

Open Access High Deductible Plan (with VEBA) offers the choice of going to an in-network or out-of-network clinics/hospitals. When treatment is in-network, the office visit co-pay is 20%, after the deductible has been met, and the prescription drug co-pay $12 for Generic/$35 for Brand formulary/$50 non-formulary. This plan has a $1,000 deductible per member/$2,000 per family. The out-of-pocket maximum per year for in-network services is $2,000 per person/$4,000 per family. Please review the plan summary for more information. All bargaining units have a Voluntary Employee’s Beneficiary Association (VEBA) benefit along with the high deductible plan. When treatment is in-network, the office visit co-pay is $25 and the prescription drug co-pay is $15 for formulary drugs/$30 for non-formulary drugs. The out of pocket maximum per year for in-network services is $1,200 per person/$3,600 per family.

Select Choice Plan offers the choice of going to an in-network or out-of-network clinics/hospitals. When treatment is in-network, the office visit co-pay is $15 and the prescription drug co-pay is $15 for formulary drugs/$30 for non-formulary drugs. The out-of-pocket maximum per year for in-network services is $1,200 per person/$3,600 per family and an out-of-pocket maximum per year for prescriptions of $500 per person/$1,000 per family.
