Health and Dental Insurance and Flexible Spending​

November 13 through November 28, 2017 at 4:00 PM

Changes made during Open Enrollment become effective on January 1, 2018​​


If you are satisfied with your current Health and Dental plan(s), you do NOT have to do anything to continue coverage into 2018

If you choose to participate in Flexible Spending, you must re-enroll each year.  Instructions will be sent from OneDigital (previously Corporate Health Systems –CHS) via email during open enrollment.

HealthPartners Health Insurance

Health Insurance Forms – 

Completed forms should be returned to "DSC: attention Benefits"

If you already have health insurance with the District and you want to:

  • Add a dependent
  • Drop a dependent
  • Change plans
  • Drop coverage
 
If you do NOT currently have health insurance with the district and want to enroll in coverage complete the Enrollment form

 

If you will have another health insurance plan in addition to your district coverage, please complete the Health Insurance Coordination form

Health Partners OE Coordination Form

 

Combined Medical Benefit

If you and your spouse hold insurance eligible positions within the district, you may be eligible to combine your health insurance benefits.

If you have questions about eligibility, please contact your benefit specialist.

An email for already enrolled participants has been sent. This form must be completed by Tuesday, November 28, 2017 for the combined benefit to continue in to 2018.  No late forms will be accepted.


VEBA Information (HRA) – High Deductible Plan only
Administered by OneDigital (formerly Corporate Health Systems)

  • Beneficiary Change form – Life Insurance Beneficiary Change Form
    • There is never an open enrollment period for life insurance
    • All supplemental life coverage applications after your initial eligibility is subject to Evidence of Insurability (EOI)
    • If interested in applying for life insurance, contact your benefit specialist
